Waiver of Liability for Free Haircuts and Styles
You hereby acknowledge that you/your child is voluntarily participating in a free haircut and hairstyling service offered by ChiGivesBack in partnership with Madison Street Barbers, & Englewood Breakroom. In consideration of being allowed to receive these services, you agree to release and discharge ChiGivesBack, Inc and its employees, partners, agents, and representatives from any and all liability, claims, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury that may be sustained by you during the course of this service.
You understand that haircuts and hairstyling services involve certain risks, including but not limited to the use of sharp tools, hair products, and potential reactions to such products. You agree to assume all risks associated with the services provided, whether known or unknown and hereby release ChiGivesBack, Inc. from any responsibility or liability for any such risks or consequences.
You further agree to indemnify and hold harmless ChiGivesBack, Inc and its employees, agents, and representatives from any and all claims or liabilities that may arise as a result of your participation in these services, including but not limited to claims for personal injury, property damage, or any other loss or damage that may occur during or as a result of the haircut and hairstyling service.
You acknowledge that you have been advised to consult with a medical professional if you have any concerns about the haircut and hairstyling services, especially if you have any known allergies, skin sensitivities, or other medical conditions that may be affected by such services.
By signing up for Labor or Love with ChiGivesBack, you give permission for us to utilize images captured during the said event through video, photo, and digital camera, to be used solely for the purposes of ChiGivesBack promotional material and publications, and waive any rights of compensation or ownership thereto.
You hereby affirm that you are at least 18 years of age, or that if you are a minor, your parent or legal guardian has read and consented to this waiver of liability on your behalf.
By signing this waiver, you acknowledge that you have read and understood its contents, and you voluntarily and willingly agree to be bound by its terms.
